If the adjuster wheel is installed backwards it will do just what you are saying is happening. Coach

That's what I've always done with drum brakes. One side at a time so you've always got an example.

If the adjuster is installed backwards, the brakes will still work but they won't self adjust. If that isn't even in there, the hardware still shouldn't fall off unless it wasn't installed properly. I've been a mechanic for many years and found out anything is possible.

There's a good post in the HOW TO section:

https://www.cherokeeforum.com/f51/ho...-8-25-a-58947/

This might help you out (except I think he put the shoes on in reverse)

The small cable that starts at the main stud goes to a finger that contacts the adjuster wheel and when the brakes wear and get loose, movement makes that finger move the adjuster wheel keeping them in spec. If the adjuster is frozen and don't turn, the self adjuster can't work.

The adjuster bracket is bent but it still doesn't look right. It should be against the wheel with pressure holding it there.

As far as the e-brake, I don't see the bar above the axle that lets the e-brake work both shoes. You pull the e-brake and the cable pulls the arm on the shoe and the transfer bar puts pressure on both shoes making the e-brake work.
