In the first pic, notice the "cup" i mentioned.. you can see the slots that sit in the dowel pins in the shifter base as shown in second pic. It might be a touch sticky, they can be a bear to get off at times. Spray some penetrant around the outside of the cup.
Again, press down and turn to release the shifter assembly, install the opposite.

The lower "collar" with the two flats on it is a locknut. You need to back it away from the remains of the **** (turn it to the right..), before you will be able to unscrew the **** (or what's left of it, turning to the left).
If you can't, you could use a grinder (or hacksaw, but it won't be easy), to cut along the remains of the **** (and the locknut if you have to). Don't worry too much about the threads on the shifter lever, cutting a slot through (parallel to the shifter lever) will still leave lots of strength to hold the new ****.
The shifter lever on my '96 is in 2 parts, there is a big coupling arrangement partway down, below the main boot, but above the inner boot (which you can only see after removing the main boot). The two halves are just pressed together. I didn't know how it came apart (I needed it apart to replace the inner boot), so I removed the whole assembly (as mentioned previously, with two fingers you can press down and rotate the ring on the transmission top, then the whole assembly comes out.). I put mine in the vice and beat it apart, but you may be able to separate that coupling by just pulling on it, or maybe pry it with a screwdriver against a vice-grip clamped on the lower half.
