My dad had this issue at the DMV. He went out back to his car and luck had it that the original owner was in the parking lot and happily signed it!

Pay the extra $120 and be done with it. The guy has no reason to help you and the state has men with guns and qualified immunity. Getting madder about it won't help or change anything.
Now you know, your friends know, your family knows, and we know. So at least there's that.

The website states specifically what must be done as far as tax forms to title an out of state vehicle in KY. As the legal types say "ignorance of the law is no excuse". We are only talking about what 6% tax? Live & learn I guess.
I've never heard of any BS forms necessary in vehicle transactions, neither has plenty of other people I've told about this. I did all I could do damn it, I crammed months worth of Jeep knowledge in in 2 weeks time to be sure I bought a good rig. I made sure everything worked, the title was legit, the vins matched on everything and it was a notarized deal. I couldn't have done any damn more. Bureaucrats SUCK is all I'm taking from this.
Could this guy be stalling trying to do some kind of scam bs? Like file for a lost title while I dick around with him trying to get this form? I'm just thinking since he's a scum bag and all he's probably scheming up something. I just can't understand any reason for someone to be such an *** otherwise. I hope he does read this so he can realize what trash he is. Thanks to all for looking into this though and offering your stories and experience, we're not all jerks at least.

But yeah the KY dmv sites are pretty unambiguous that this form is needed for out of state purchases.
I didn't go web searching for rules to buy a vehicle before hand. Signed and notarized title is all that should be needed. Putting someone in a difficult position requesting you reconnect and require something from the selling party is stupid.

To the OP, I'm from KY too. I found out the hard way when I was much younger about the tax codes and title stuff. Your best bet now is to pay the extra cash. It's not that much in the grand scheme of things. Especially of you consider you haven't driven the vehicle in a month.
In metro Louisville if you register a car from out of state the sheriff office will perform a full inspection. In outlying countles, they do a simple VIN match and mileage check.
KY has had that form for out of state taxes for at least 15 years that I know of. It's annoying as hell but just chock it up to knowledge. You'll not make this mistake again.
I doubt the dude is trying to scam you at this point. He probably just doesn't care about your $200
