R-12 to R134a

To do this change would I have to change the whole system or could I just get a fitting for the old system

new compressor dryer and liguid line flush all old stuff out ,is your old system shot there is a r12 replacement you can buy its called freeze 12 ebay sell it no license requiredhttp://motors.shop.ebay.com/i.html?_..._nkw=freeze+12

Well that I'm not sure about. When I turn the ac on it blows cold for about a second then blows warm air. So I was thinking of trying to see if a recharge would work and if not just swap the system

Yeah I see it all the time at the store...changeover without changing everything just mean you'll put off changing everything till later. Better to get a compressor kit and do it all now.
