Hard shifting/not wanting to shift

Hard shifting/not wanting to shift
So I've had this problem for a little while and it's seeming to get worse. But every time I get to about 25mph I don't know what's going on but my rpms jump up and she doesn't shift. But if I let off or put it down she'll shift. No real clue what to do now. Changed my trans fluid, oil, and diff oil.
