they come as part of the shock, if you snap stock pins, bar pin eliminators would be cheaper than new shocks.

just stock bolts. I'd be prepared in the rear to take an air hammer or a sledge and center punch to those top bolts when they break off, and drop new bolts through the holes and put nuts on them.

I think I used like M8's, just take one of your new shocks to lowes with you and see what size bolt fits through the holes easy.

Yeah, not necessary, but nice.
I made my own for free by using the rear swaybar brackets flipped upside down. The boltholes line up like they were made for that purpose. They work great.

More or less. I'm talking about the D-shaped brackets. The top holes line right up with the upper shock studs, and the side holes line right up with the eyelet on a bar pin-less shock.
Now, on mine, I did have to use a vise and a prybar to bend the tabs on the D-bracket out slightly so that I could fit my upper shock eyelet into them, but I think that's because mine were bent slightly inward to begin with. Otherwise, no mods are necessary.
