Clunk and lunge when put into gear

Clunk and lunge when put into gear
My 1998 jeep cherokee classic seems to make a clunk and lunge when I put it into gear. It lunges worse when I put it into 4 wheel drive and then shift from neutral to drive. Also it is a little tight shifting from park to reverse. Any suggestions?
