I would suspect the TCM is faulty.

Okay, according to Haynes, there is no separate TCM for the 42RE after 1996. Then it's controlled by the PCM. Check all the cable connections to the PCM. There may be a loose one.

When was the last time fluid and filter were changed?
Are you getting any TCM codes?
Get the codes scanned and defined.
Because of your stuck in 2nd experiences (thats limp mode), I'd be suspicious of internal solenoid pack. Believe there are three in the pack. OEM sells them only as a set. Check around for best price. Not hard to change once the pan is off but you'll also want to do fresh fluid and filter.
