slips out of gear

i drive a 99 jeep cherokee limited and just crusing in 2 wheel drive would randomly slip out of gear so id put it in nutrl and back and drive and it would go back in gear.. now its gotin worse and it will only drive in 4wheel low .. 4wheel high and 2 wheel just grinds while in gear .. has any1 had this problem?
