p0155 and p0455 after new o2 sensor

p0155 and p0455 after new o2 sensor
p0155 again I replaced the o2 sensor that is for cylinder 4-6 before cat. I thought that this was bank 2 sensor 1. Can someone enlighten me as to which sensor it actually is? I also have a p0455 EVAP leak. Recently I changed the injectors to the 784's and replaced the o2 sensor. Any ideas?
