Does P0138 mean I need an O2 sensor exactly?

Default Does P0138 mean I need an O2 sensor exactly?

So I went to Autozone and they pulled the code P0138, it says it could be a bad downstream O2 sensor. Could a bad O2 sensor be the only cause to throw this code? If not, what are my options? Please Help! Thanks

other options could be an exhaust leak and the O2 is not mesuring correctly because of it.
