NAPA
for the back: BK 6301076
link: http://www.napaonline.com/Search/Det...+50038+2038004

I could not fin the exact one for the front, which is just an lbow tube, so I installed in mine this one: BK 6301078
link: http://www.napaonline.com/Search/Det...078_0284191361
the "problem" is that it's a bit higher than the one you have and it's very close (almost touching) the hood when closed. I turned mine 180 degree around (so the elbow is pointing to the back) so there is a bit more clearance "under the roof" that way ...works just fine.

and get pair of new rubber grommets for sure, as the old one gets hard and you may actually have to destroy them or dent while removing ...they are tough to get out. Good luck

grommets: BK 6301410

ps when clicking on napa links you may hit the page where you may need to click "shop on line" to pass that page and get to the product...

arghhhhhhhhh, just did napa site search again, but did it not by searchnig a "part name" but instead went through their "search by job" thing and when went through "valve cover gasket replacement" found this:
http://www.napaonline.com/Search/Det...+59999+3999999

so , this one looks exactly as you'd need for the front, p# is CRB 29574

I wish I found this before

btw, napa names this part as "PCV Valve" , well, that's just the way it is. I've found in the past some not so "correct" naming for some parts here and there, which makes it hard to find by "key word search".

again, this is just an open tube fitting, but it's a low profile one.
as I see at the picture you have different air intake and your tube has straight (not 90degree angle) fitting connector, so you only can have the elbow to be possitioned one way, as is on the photo... well, blah blah blah ...the point is that you'd better order the low profile elbow for the front. You may still be OK with the other one, I don't know.
Also I have my underhood liner removed so I have enough clearance for the higher elbow, not sure if it will work for you.

@ Dr Zee "and get pair of new rubber grommets for sure, as the old one gets hard and you may actually have to destroy them or dent while removing ...they are tough to get out. Good luck "

I agree Dr,but how do you remove it? I tried prying it with flat screw driver & they broke, its already brittle(of course, its exposed to heat) so I stopped removing. will I just continue doing/breaking it then replace with a new one? is there something I need to take note of?
Thank you.

The name is ,has been, and always will be hit or miss with people. Jeep calls it a CCv system ,but some stores, books, people call it a pcv system, because of the old systems of yesterday. I use CCV cause the dealership calls them that.

Well, I can't give any "HOW TO DO IT RIGHT" advice as I simply have no clue. All I can tell is how I did it, and it was not "pretty" heh heh...
And honestly after looking closer into how the fittings and grommets are constructed plus adding to the picture the fact that the rubber gets hardened - I don't think there is an easy way to remove them without damaging plastic elbows or grommets or both.
I removed mine destructively.
I saved the old grommets and the front elbow (who knows why ...huh) , but with those pieces I could sort of recreate the removal process and made some close up photos, will post the pictures in a few mins. Hold on ...

ok
here are the close up pictures of the front elbow fitting and the grommet.
As you see , the fitting has very sharp flange, so as it is inserted into the grommet, it gets locked, it's like "one way ticket" , well, if the grommet new and flexible, then it is still possible to remove the fitting, but with hardened rubber , I'd say - no way.
