Default break fluid disappearing????

My brake light on my dash will come on every once in a while and I go look at the brake fluid holder and its nearly empty...I add some fluid and the light goes off and everythings back to normal for awhile. When the light comes on my brakes start to feel real soft like I have to push the pedal in more to get the brakes to engage. I also noticed some fluid on the passenger rear tire. Whats the problem?
