new parts fail
So I just had gears lockers and shafts installed i. I drove for less than two hundred miles following all break in procedures and then one one of my rear axle shafts came out pulling it apart revealed my bearings failed has anyone else had this issue if so what's the cause

What rear axle do you have? Did the c-clip fail and let the axle come out, or did the flange bolts all strip/come loose to allow it to come out?
Which bearing? Carrier or outer axle bearing? If it's carrier, I would suspect a poor installation with either too much or too little preload? Was there any gear whine or noise?

Have you checked the new bearing to see if they are the same diameter (outside) as the old. They fit the axle side but if for some reason they are a slightly different size on the outer the they can slip through the retaining plate and check the plate for damage also. Some pics could possibly help with more diagnosis.
