Shift linkage?

Shift linkage?
Ok so the "Slug" keeps jumping out of 2wd when i go to take off from a stop and I cant get it to go into 4Lo. Would this be a linkage problem? If so where can i find a replacement one and is there a procedure to making sure its hooked up correctly?
