Output speed sensor

Default Output speed sensor

So I have a question for you all. I had some issues with my trans a month or so ago. Changed the gov pressure sensor, filter, and fluids, vss, and output speed sensor only to have my 42re not shift into 4th. The rest of the gears are fine. So I pulled the OSS to try an old one to see if it will change. Well I found that the tip of my less than a month old sensor was melted. Any help or ideas are appreciated. Sorry the pic sucks had trans fluid on the lens.
96 with a 4.0, 42re and 242 TC

Was it leaking internally too? I just cant seem to find one that won't leak. But I'm not burning.
