Default Upper control arm bushings wont stay in??????

I just replaced all my control arm bushings. They were all press out old, press in new except the axle side uppers. I pressed the old ones out with a ball joint press but the new ones I just banged in with a hammer (took some pounding and seemed very tight fitting). I took it wheeling a few times and now the upper axle bushings are working there way out. You can see the little flange on the bushings that once was flat against the mounts. I tried tack welding them in place but they broke the welds and slid out again. Anyone have any experience with this???? Also I am running RE control arms but only the lower axle side has flex joints, Could this be the cause????? Thanks for any help. Sorry for cell phone pics.

That's how mine look....and we just R&R'd them. I believe they're supposed to be centered up like that.

The one on the drivers side will not go in so it sits flush. The pass side will go all the in flush with bracket(towards inside of axle).
