Default Rough shake/jerk if I take off to fast

My '95 is a standard and it has this odd, really rough jerk if I take off to fast.

If I let the clutch out really slowly and give it a good bit of gas, it does ok (even though it still jerks a little in upper gears).

If it comes out of first gear wrong (as in it shakes) when I first start it, it will shake the entire time that it is running. However, if it comes out of first without the shaking, it will run fine until I turn it off.

It is on and off regularly. I could drive to the store in the morning and it would be really jerky, then go somewhere else later in the day and it run perfectly fine.

I live in south Louisiana where it rarely gets below 50 degrees so I don't think temperature has much to do with it.

Sorry I can't give more details, it is really hard to explain. I have NO clue what is causing it but it really concerns me.
