Blend door actuator is stuck. This can be because of a broken or cracked vacuum line or an actual obstruction for the blend door. 99% of the time is the vacuum tubing either broken, or disconnected

Under the dash you'll see a mini diaphragm. Check to see the tubing is connected. Pop the hood trace the vacuum line back from the vacuum ball reservoir behind your front bumper. Look for any disconnects or broken pieces. You'll want to follow this to the to the IM and still continue to look for breaks or disconnects

Check out the vacuum line off the vacuum reservoir. Check it all the way back to the intake, checking for leaks or broken tubing. This pic shows where to find the line.

Here's some pics of the vacuum system in your engine bay. Click on each photo to enlarge it.

Your blend-air door actuator is a servo motor operated device, not vacuum operated.

small rubber hose and slide both ends into it. And until your heater cores hooked up/replaced wont get heat.

There's a photo in that link I posted that shows how to fix that broken tube.

O'reilly Auto Parts has the small tubing for that.
