I ordered stickers & signed up on the forum as a member. When can I expect my CF stickers????

I know its not a big deal but I paid for them so I'm kinda looking for them to show up.

I see now that there is a form to fill out (Done). But why do we fill out a form? Dont they get that info upon making the Payment transaction... I dont understand the disconnect.

What form? I ordered on 5/2/14 and haven't received them. I paid through PayPal and I didn't see a form
