Poor Shifting

Engine: 5.2L V8
Poor Shifting
My mom has a Pontiac Grand Prix. I don't have a whole lot of details on specs but I can walk outside and find out if it's relevant.
Now when you first start the car it usually it shifts fine. On warmer days it tends to start acting up faster. It's as soon as the vehicle gets to operating temperature that it acts up.
The gears don't seem to shift properly. It'll rev up to 2500 RPMs and sometimes higher. If you let of the pedal it seems to shift half way decent but if you hold the pedal you can hear an audible sound that is not normal when it shifts.
Any help or advice would be appreciated.
