Decrease in RPM when holding the throttle smooth?

Decrease in RPM when holding the throttle smooth?
My 5.2 is losing 250 rpm when I'm crusing at any speed and then regains the lost RPM and surges like on and off again throttle resonse. It does this about 4 seconds... I have changed the TPS and plugs. Anyone ever run into this issue?
