Default Upper control arm bushing help

I had the control arm bushings changed out on my '00 XJ maybe 6-8 weeks go. I noticed recently that the steering feels loose and there is a creak coming from under the front end. I looked today and this is what I found. WTH happened????

Possibly the bolt was torqued with the wheels off the ground? That would destroy the rubber. Not sure why the arm would slide on the bushing unless the hole through the arm was messed up.

Or what I meant to say was not sure why the bushing would slide in the mount on the axle. Lol.

Gonna run it into the shop tomorrow and see if they can shed any light on it

It looks like it wasn't pressed all the way in. It should look like this.

Looks like whoever installed that bushing, beat on it with a hammer. That is how not to install them. Must be pressed in. Can be put in freezer to shrink it and make it easier.
Beating on the 'can' of bushing will distort it, then it will not fit properly.
