I don't have the write up but the one I read they use a wheel adapter as a pattern

they had a tall drill press drilled a hole the size of the axle in a 3/4 plywood
clamped it the the table to hold it the right distance off center and then just turned it to line up with the adapter holes

the bolt pattern on my axles right now are 5 bolt at 5.50 and it needs to be 5 bolt at 4.50 i also have a back up set of axles for the new 12 bolt but they are 6 bolt im gonna redrill them just for back up.. im also redrilling the drums

if you can find a machine shop with a cnc mill they can do it in ten minuets

find a cj d30 and swap the knuckles and hubs then you will be 5 on 5.5
all around and have locking hubs

why not just get some adapters to go from the 5x5.5 to the 5x4.5. if you don't want to do that take it to the machine shop that way it will be done correct the first time

why are you runing a 12 bolt? it seems like its not even worth the time making it work. there are plenty of axles out there already w/ the bolt pattern you need, no need to have to redrill shafts and drums
