Default New Heater Core Leaking?

I have all the symptoms of a failing heater core. I replaced about a year ago, so it doesn't seem like it could have failed this quickly. Is there anything else that could cause these symptoms (coolant on floorboard/coming out of vents)?

Could this cause coolant to get in the heater box?

Sure could. Back the hoses away from the firewall a bit to where you can see the heater core inlet and outlet pipes and then tighten them.
