could be the vac disconnect motor, or the gears inside the transfer case

Crawl under your rig and take a look at the linkage. Look for bare metal on the lods. This will indicate if the set screw (bolt, actually) is loose.

SHIFT LINKAGE ADJUSTMENT
(1) Shift transfer case into 4L position.
(2) Raise vehicle.
(3) Loosen lock bolt on adjusting trunnion (Fig.
87).
(4) Be sure linkage rod slides freely in trunnion.
Clean rod and apply spray lube if necessary.
(5) Verify that transfer case range lever is fully
engaged in 4L position.
(6) Tighten adjusting trunnion lock bolt.
(7) Lower vehicle.

I've heard that problem could be caused by worn pads on the shift fork inside the tcase.

Yah, mine does the same, but I've noticed it usually only does it if I go from 4L to 4H. When I go from 2H to 4H its usually fine unless I pull up too hard on the shifter. It's like its slipping in between 4H and N.
