AC low side pressure over 100 psi and now condenser replacement assistance

Model: Grand Cherokee
Engine: 4.7L
AC low side pressure over 100 psi and now condenser replacement assistance
I just replaced my evaporator and accumulator, got the AC evac and charged, had cooler (not cold) air for 2 days and now nothing. I put a gauge on the low side line and it shot up over 100 psi, without fully connecting the gauge. Also, there's now a small hole in the condenser slowly releasing pressure (small chunk of ice at location of hole and hissing).
Two questions, what failed component(s) could be causing the pressure to be so high in the system.
And, can someone send me instructions for replacing the condenser.
Some other info, I could hear the compressor engaging but it didn't sound good while doing so. Both lines to/from the evaporator were very hot to touch, as well.
edit: forgot to add car info - 99 grand cherokee limited with 4.7 engine

Did you ever figure this out? Mine is reading ~135psig. In the past two years I have had both coils replaced and the compressor, now this. I don't have the noise in my compressor though. Any thoughts?
